Case Manager

Program Supervisor

SUMMARY:
Provide case management/social work services to clients of Saint John's Hospice Residential Program according to social service standards of Catholic Social Services, Saint John's Hospice Mission Statement and the requirements of contracting agencies.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
· Complete intake paperwork for new clients and provide program orientation.
· Conduct thorough assessments for each assigned client.
· Develop comprehensive service plans tailored to identified needs, outlining goals, action steps, and realistic timeframes.
· Ensure accurate entry of all client data into HMIS and Harmony databases.
· Initiate weekly meetings with new clients, adjusting frequency as needed during the initial month, transitioning to bi-weekly meetings thereafter.
· Monitor client health and medication adherence, maintaining updated medication logs.
· Oversee the collection and testing of urine samples.
· Document client participation in the Savings & Fees Program.
· Regularly review progress on service plan goals, offering assistance as required.
· Monitor client compliance with treatment programs.
· Assist with completion of housing applications.
· Maintain meticulous case files for all assigned clients.
· Provide individual counseling within scope of competency.
· Facilitate resident RAP group meetings according to schedule.
· Attending weekly or scheduled meetings with the Case Management Team.
· Meet with supervisor as scheduled for guidance and support.
· Coordinate with external agencies to ensure comprehensive client care.
· Cultivate and manage a comprehensive collection of resources and referral sources, connecting clients with appropriate services.
· Maintain flexibility in work schedule to accommodate occasional evening and weekend activities.
· Participate in relevant training sessions and scheduled meetings to enhance professional development.
· Compile statistical data on program outcomes for regular reporting to Catholic Social Services and OSH.
· Fulfill additional duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
· Computer, and typing skills.
· Knowledge of homelessness, substance abuse, and mental illness preferred.
· Good written and verbal communication skills
EDUCATION:
· Bachelor's Degree in Social Work, Human Services, or Behavioral Health.
EXPERIENCE:
· Two years social work experience
PHYSICAL REQUIREMENTS:
· Ability to stand, sit, walk, climb stairs, operate controls, reach out with hands, occasionally lift objects up to 40 lbs.
